How to apply

Application checklist
The following materials must be included in the application:

Form 1: General application form with
• Four-digit identifying team code
• Name of faculty adviser
• Name of outside professional adviser (if applicable)
• Signature of sponsoring school/department/program head verifying that all participants
  are students enrolled for spring 2005
• Designated student team leader, contact E-mail address, and delivery address

Form 2: Team Member 1—Leader
• Team statement
• One-page curriculum vitae

Form 3: Team Member—as many as appropriate
• One-page curriculum vitae for each team member

Deadlines
All applications must be received in the ULI offices by 5 p.m., e.s.t., Friday, December 10, 2004. No notification of receipt by ULI will be sent to applicants; therefore, it is recommended that applications be faxed or sent by registered mail, FedEx, UPS, or other means that allows the sender to track the status of the shipment. Address any inquiries to udcompetition@uli.org. No phone calls, please.
